The national immunization program continues to face social and religious barriers that influence vaccine acceptance rates in Indonesia. Communities with high levels of religiosity tend to consider the legitimacy of religious leaders and the halal status of vaccines before participating in immunization programs, creating challenges for the government in building public trust in health policies. This study aims to analyze the role of religious leaders in increasing public participation in immunization and to explain the relationship between religious legitimacy and vaccine acceptance within the context of public health policy in Indonesia. The study employed a qualitative method with a library research design through the analysis of scientific journals, health regulations, religious fatwas, and various academic documents relevant to the research topic. The findings indicate that religious leaders play a significant role in shaping public perceptions of vaccination through moral legitimacy, social communication, and the strengthening of trust in government health programs. The study also reveals that religious fatwas and collaboration between the government and religious institutions can reduce vaccine hesitancy and improve the effectiveness of health communication in religious communities. This study concludes that the integration of religious leaders into public health strategies constitutes an important factor in improving the success of national immunization programs in Indonesia.
